About Oklahoma's Vaccine Policy

  We'd like to reiterate the vaccine laws in Oklahoma, specifically the exemption provisions relating to public schools and DHS licensed daycare facilities. In the State of Oklahoma, there are currently three different forms of exemptions available: medical, religious, and philosophical.
  Any minor child, through his or her parent or guardian, may submit to the health authority charged with the enforcement of the immunization laws:
  1. a certificate of a licensed physician stating that the physical condition of the child is such that immunization would endanger the life or health of the child;
  2. or upon receipt of a written statement by the parent or guardian objecting to such immunizations because of religious or other reasons, then such child shall be exempt from the provisions of this act.

  Please provide ODH form 216-A, Certificate of Exemption, issued by the Oklahoma Department of Health (ODH) to all patrons. To obtain copies of form 216-A, please see your local Health Department office.

  If your school does not have this form 216-A on site, accept a copy from parent until the official form can be supplied by the school.
